For Our Patients

Healthy Partnerships

Healthy partnerships lead to higher quality and better results. That’s why our providers place the patient at the center of everything we do.

Our vision of patient centered care means that our patients are active participants in the delivery of care that emphasizes wellness through proactive planning and implementation. This approach helps our patients control their chronic conditions to avoid critical situations as much as possible.

Proactive care planning and chronic disease prevention are important reasons why North Central Arizona Accountable Care should be your first choice for quality healthcare.

The Right Care. When You Need It.

Providers who have joined North Central Arizona Accountable Care have made a commitment to collaborate at a high level to provide you with advanced care that meets strict standards for quality and performance. Supported by sophisticated electronic medical record technology, our providers communicate with you, and with each other, to ensure that you get the care you need when you are sick as well as the support you need to stay healthy and well.

Accountable Care Organizations (ACOs) are very different from more restrictive healthcare models such as Health Maintenance Organizations (HMOs). Unlike an HMO, an ACO can’t tell you which healthcare providers to see and can’t change your Medicare benefits. If your physician participates in an ACO like North Central Arizona Accountable Care, you always have the right to choose any provider that accepts Medicare at any time.

Other benefits include:

  • Your physician has a demonstrated commitment to deliver high-quality care and is expected to meet Quality Criteria as defined by The Centers for Medicare and Medicaid Services.

  • You can avoid unnecessary tests and duplicative services as a result of higher levels of interaction and information sharing between your providers.

  • Your physician will focus on preventive care.

  • Your physician will work closely with you to help you control any chronic illnesses you may have.

  • Your physician will be your partner in your good health.
